As a chronic over-holder, I can add that some people feel an obligation to rush toward the door if they are kind of far away and you are waiting for them with the door. This adds even more awkwardness.

Hold time (in seconds) = W + P/D + G*(X-1)

W = Door weight (measured in stones)
P = Pace of next person (measured in furlongs per fortnight)
D = Distance between arriving person and door (in mm)
G = Gender of next person (female=1, male=0)
X = Hotness of next person (scale of 1-10)

Holding the door is a courtesy, not an obligation. I wouldn't feel bad about not doing it, especially in an age where people are touchy about equal rights.

I say that as someone who almost always holds the door. It's a nice thing to do. The world could stand to have a little more nice in it.

The only exception would be if you were enough of a sucker to make eye contact once inside with someone walking toward the elevator from a greater distance. In which case you simply pretend to press the DOOR OPEN button while actually banging DOOR CLOSE.

I'm with KGB. Unless I'm really in a hurry or feeling antisocial, in which case, I'll do everything in my power to pretend I didn't see them and get out of the radius of direct eye contact before they make it to the door.

is the amount of time that it takes for the door to close if you swing it all the way out as you enter. Assuming it's at least a beat or two longer than that, you're off the hook. So it shall be written, so it shall be danced.

I may be the weird one here, but I tend to view someone waiting a little too long to hold a door for me as more detriment than benefit. And I say that as a proud, card-carrying door-holder.

I basically motion them to go ahead. If I am in a hurry it is a rare instance when I let the door slam. I do feel bad - but - fuck em, I am hungry.

Funny too, when it is a nice looking lady, I feel like a creeper, but I do it anyway.

I almost always look behind me to make sure the door is not slamming on someone. I hate when I slam a door on someone.

EDIT: Like all of life, it's conditional and humanistic. If you look and see somebody and they have both hands full, hold the door even if they're 15-20 paces back. If you look and they're 20 paces back lollygagging, let the door shut. If they are 10 paces and racing, hold the door. If they catch your eye and smile, hold the door. If they fired your friend a few years ago and they're 5 paces back, pull the door shut on them.
